Roles & Responsibilities

• Assess patients’ mobility, strength, and functional needs, then design individualized treatment plans
• Deliver hands-on therapy and therapeutic exercise programs to improve patient outcomes
• Educate patients and families on home programs and safe mobility strategies
• Collaborate with interdisciplinary teams to coordinate effective, patient-centered care
• Document evaluations, progress notes, and discharge plans accurately in the medical record

Qualifications

• Current Physical Therapy license for the state of California (required)
• BLS (Basic Life Support) certification (required)
• Epic experience preferred (electronic medical record familiarity is a plus)
• Willingness to complete Trinity skills checklist and standard pre-employment health screenings
• Ability to participate in monthly on-call rotation

Knowledge & Skills

• Strong clinical assessment and therapeutic intervention skills in physical therapy
• Experience with care planning, progress tracking, and discharge planning
• Comfortable documenting in electronic medical records (Epic preferred)
• Excellent communication and patient education skills
• Team-oriented approach and ability to coordinate with nurses, physicians, and rehab staff
