Roles & Responsibilities

• Prepare, verify and dispense medications accurately and safely
• Collaborate with physicians, nurses and the care team to optimize patient therapy
• Provide medication counseling and education to patients and staff
• Monitor medication use for interactions, allergies and appropriateness
• Manage pharmacy inventory and ensure proper storage and handling
• Participate in medication safety and quality improvement initiatives
• Maintain accurate patient and pharmacy documentation in line with hospital policies

Qualifications

• Active Pharmacist license in Texas or ability to obtain prior to start
• Minimum 1 year of pharmacy experience (acute care/hospital experience preferred)
• Doctor of Pharmacy (PharmD) or BS in Pharmacy (PharmD preferred)
• BLS (Basic Life Support) certification or willingness to obtain
• Strong clinical judgment, attention to detail and excellent communication skills

Education

• Doctor of Pharmacy (PharmD) preferred; BS in Pharmacy acceptable

Knowledge & Skills

• Solid understanding of hospital pharmacy operations and medication safety
• Experience with IV/sterile compounding and unit-dose dispensing
• Medication reconciliation and formulary management experience
• Comfortable working in fast-paced acute care settings and collaborating with clinical teams
• Good documentation practices and familiarity with electronic medical records
