Roles & Responsibilities

• Assess resident functional abilities and create meaningful, goal-focused OT plans
• Implement individualized treatment to improve independence and daily living skills
• Collaborate closely with the interdisciplinary team to deliver coordinated care
• Document treatment progress accurately and in a timely fashion
• Support patient and family education to promote safe transitions and home strategies
• Meet productivity expectations (90% productivity requirement) while maintaining quality care
• Follow CRG policies & procedures and represent the facility professionally (black scrubs dress code)
• Be present on-site to receive guaranteed hours and ensure continuity of care

Qualifications

• Licensed Occupational Therapist (state licensure required; ability to obtain NM licensure if not already licensed)
• 1-3 years of OT experience (experience in a Skilled Nursing Facility/SNF preferred)
• Strong clinical reasoning and hands-on treatment skills appropriate for geriatric and post-acute populations
• Comfortable with documentation practices and productivity expectations
• Professional presence and ability to work collaboratively with an established therapy team

Education

• Degree in Occupational Therapy (required for licensure)

Knowledge & Skills

• Skilled Nursing Facility (SNF) therapy experience and familiarity with post-acute care workflows
• Patient-centered treatment planning and ADL retraining
• Good clinical documentation and time-management skills
• Strong communication and teamwork — you’ll work with nurses, aides, families and leadership
• Ability to follow facility policies and maintain a professional appearance (black scrubs)
