Roles & Responsibilities

• Evaluate patients and develop individualized occupational therapy treatment plans tailored to SNF residents
• Implement therapeutic interventions to improve daily living skills, mobility, and independence
• Document assessments, progress, and treatment plans clearly and timely in accordance with facility and CRG policies
• Collaborate with an interdisciplinary rehab team to coordinate care and optimize resident outcomes
• Educate residents, families, and staff on interventions, home programs, and safe transfer techniques
• Maintain productivity expectations (90% productivity requirement) while delivering high-quality care
• Adhere to facility protocols including dress code (black scrubs) and attendance requirements to ensure guaranteed hours

Qualifications

• 1-3 years of occupational therapy experience (as listed in the job posting)
• Current or eligible Occupational Therapist licensure (state license as required)
• Experience working in a skilled nursing facility (SNF) setting preferred
• Ability to meet a 90% productivity requirement and follow CRG policies & procedures
• Reliable onsite presence (therapist must be in the building to receive guaranteed hours) and professional presentation (black scrubs)
• Strong communication skills and ability to work well within an interdisciplinary team

Education

• Degree in Occupational Therapy (OT)

Knowledge & Skills

• Skilled in OT assessments and treatment planning for SNF/residential populations
• Knowledgeable in documentation standards and productivity tracking
• Familiarity with skilled nursing facility workflows and interdisciplinary collaboration
• Ability to prioritize caseloads, manage time effectively, and meet clinical targets
• Comfortable following company policies and participating in regional interviews/meetings
