Roles & Responsibilities

Evaluate patients to determine functional status and therapy needs
Develop individualized treatment plans that promote independence
Implement therapeutic interventions focused on activities of daily living
Collaborate with nursing and interdisciplinary teams to coordinate care
Document progress and outcomes clearly and timely
Educate patients and families to support carryover and safety at home
Participate in care planning and discharge preparation to ensure smooth transitions

Qualifications

Licensed Occupational Therapist or license-eligible in Minnesota
New graduates are welcome, any level of experience accepted
Comfort working in a Skilled Nursing Facility environment
Ability to work day shift and accept overtime when needed
Strong communication and teamwork skills

Education

Degree in Occupational Therapy from an accredited program, new grads welcome

Knowledge & Skills

Skilled in occupational therapy assessment and intervention
Experience or familiarity with SNF workflows and documentation
Patient-centered care and skilled patient handling
Care planning, progress documentation, and outcome measurement
Good clinical reasoning, creativity, and problem solving
