Roles & Responsibilities

• Assess patients’ functional abilities and occupational needs in a skilled nursing setting
• Develop individualized treatment plans focused on meaningful daily activities
• Implement therapeutic interventions to improve independence and safety
• Collaborate with nursing and interdisciplinary teams to coordinate care
• Document progress and outcomes clearly and timely in the patient record
• Educate patients and families on strategies for daily living and safe transitions
• Support safe discharge planning with practical, patient-centered recommendations

Qualifications

• Any experience level welcome — including new grads
• Degree in Occupational Therapy (or completion of an accredited OT program)
• Licensed or eligible for Occupational Therapy practice in Illinois (facility will verify licensure requirements)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Strong clinical reasoning, documentation, and communication skills

Education

• Degree from an accredited Occupational Therapy program (per facility requirements); new grads accepted

Knowledge & Skills

• Skilled in occupational therapy assessments and intervention planning
• Experience or knowledge of SNF workflows and regulatory documentation
• Ability to collaborate with interdisciplinary teams
• Proficient documentation and clinical outcome tracking
• Patient- and family-centered teaching and discharge planning
