Roles & Responsibilities

• Assess patients’ functional abilities and design individualized OT treatment plans
• Provide skilled therapeutic interventions to improve daily living and mobility
• Collaborate with nursing and interdisciplinary teams to coordinate care
• Document patient progress clearly and maintain accurate clinical records
• Educate patients and families on strategies to maximize independence and safety
• Adapt treatment approaches to meet the needs of a diverse caseload

Qualifications

• Valid SD Occupational Therapy license or eligibility to obtain state licensure
• OTR/L credential preferred (or equivalent certification as required)
• Current CPR/BLS certification (or willingness to obtain)
• Comfortable working in a skilled nursing/long-term care setting
• Strong communication, organization, and teamwork skills

Education

• Degree in Occupational Therapy (as required for state licensure)

Knowledge & Skills

• Skilled in ADL training, adaptive equipment recommendations, and home safety evaluations
• Experience with documentation, care planning, and interdisciplinary collaboration
• Ability to assess mobility, cognition, and functional status with evidence-based methods
• Good clinical reasoning and the ability to educate patients and caregivers effectively
