Roles & Responsibilities

Provide compassionate direct patient care, monitoring vital signs and reporting changes promptly
Administer medications and treatments accurately while following provider orders and facility protocols
Collaborate with nurses, providers, and interdisciplinary team members to coordinate patient care plans
Document patient assessments, interventions, and responses in the electronic medical record clearly and timely
Assist with patient admissions, discharges, and transfers ensuring a safe, comfortable experience
Educate patients and families on care plans, medications, and follow-up instructions with clarity and empathy

Qualifications

Current LPN license valid in Pennsylvania (required)
Basic Life Support (BLS) / CPR certification (or willingness to obtain upon hire)
Prior practical nursing experience in a clinical setting preferred
Strong clinical assessment skills and ability to prioritize patient needs
Clear background check and relevant health screenings as required by facility

Education

Graduate of an accredited Practical Nursing (LPN/Vocational Nursing) program
High school diploma or equivalent required

Knowledge & Skills

Medication administration and documentation best practices
Basic nursing assessments and wound care
Familiarity with electronic medical record (EMR) systems
Effective patient and family communication skills
Ability to work in a fast-paced healthcare environment with attention to detail
Teamwork and collaboration across disciplines
