Contract Type: Per-Diem

Shift Type: 8-hour shifts; specific start/end times and days are not specified in the posting and will be confirmed with the hiring team

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care and support daily living activities
Administer medications and treatments according to established protocols
Monitor patient condition and accurately document changes in charting systems
Communicate effectively with RN leadership, physicians and the interdisciplinary team
Assist with wound care, IV monitoring and other clinical tasks as assigned
Promote a safe, clean and therapeutic environment for residents and patients
Support care transitions, including admissions, discharges and care planning

Qualifications

Valid LPN license in Pennsylvania or eligible to obtain PA licensure
Current Basic Life Support (BLS) / CPR certification
Clinical experience as an LPN in long-term care, skilled nursing, or similar setting preferred
Strong communication and teamwork skills with a patient-centered mindset
Up-to-date immunizations and ability to pass background and health screenings

Education

LPN diploma or vocational nursing certificate

Knowledge & Skills

Medication administration and documentation best practices
Basic clinical assessment and vital signs monitoring
Electronic medical record (EMR) charting and accurate documentation
Infection control standards and safe patient handling
Time management and prioritization in a fast-paced clinical setting
Empathy, patient advocacy and strong interpersonal skills
