Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents and patients
Administer medications accurately and on schedule, following facility protocols
Monitor vital signs and patient condition, reporting changes promptly to the care team
Document patient care clearly and timely in electronic or paper charts
Collaborate with nurses, CNAs, therapists, and providers to ensure coordinated care
Support resident comfort, dignity, and activities of daily living with respect and empathy
Participate in care planning and communicate effectively with families

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania required
Graduation from an accredited LPN/LVN program
Current CPR/BLS certification preferred or obtainable upon hire
Comfortable with medication administration and basic nursing assessments
Strong communication skills and a team-oriented attitude
Prior long-term care or skilled nursing experience is a plus

Education

Completion of an accredited LPN/LVN program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation
Basic wound care and infection control practices
Patient assessment and monitoring of vital signs
Familiarity with electronic health records or willingness to learn
Time management and prioritization in a busy care setting
Compassionate patient-centered care and strong interpersonal skills
