Roles & Responsibilities

Provide compassionate, patient-centered nursing care to assigned residents
Administer medications and treatments per provider orders with accuracy
Monitor and document vital signs and changes in patient condition
Collaborate with RNs and the interdisciplinary team to coordinate care
Maintain accurate, timely clinical documentation in the facility charting system
Assist with admissions, discharges, and patient education to support transitions

Qualifications

Current Licensed Practical Nurse (LPN) license valid in Pennsylvania
Current CPR/BLS certification or willingness to obtain upon hire
Able to pass required background check and drug screening
Strong communication, observation, and teamwork skills
Compassionate approach to patient care and attention to detail

Education

Graduate of an accredited Practical Nursing program or equivalent

Knowledge & Skills

Solid knowledge of basic nursing care and patient assessment
Medication administration and safe documentation practices
Infection control procedures and wound care fundamentals
Familiarity with electronic medical records and charting
Good time management and prioritization in a busy clinical setting
