Roles & Responsibilities
- Provide compassionate direct patient care that promotes dignity and comfort
- Administer medications and treatments safely and accurately
- Perform thorough patient assessments and chart findings in the electronic record
- Collaborate with nurses, CNAs, therapists, and providers to deliver coordinated care
- Monitor patients closely and promptly report significant changes to the care team
- Assist with admissions, discharges, and transfers to ensure smooth transitions
- Adhere to facility policies, infection control practices, and safety protocols
Qualifications
- Current LPN license in Pennsylvania (required)
- Completion of an accredited practical nursing program
- Current CPR/BLS certification (or willingness to obtain)
- Strong clinical judgment and attention to detail
- Excellent communication and teamwork skills
- Previous LPN experience preferred but motivated new graduates are welcome to apply
Education
- Graduate of an accredited practical nursing program
- Current LPN licensure in Pennsylvania
- CPR/BLS certification
Knowledge & Skills
- Medication administration and documentation
- Basic wound care and dressing changes
- Patient assessment and vital sign monitoring
- Familiarity with electronic health record documentation
- Infection control and safe patient handling
- Strong interpersonal skills and patient-centered care approach
