Roles & Responsibilities

Deliver compassionate, patient-centered care at the bedside
Administer medications and treatments precisely and safely
Monitor and document patient vital signs and changes in condition
Coordinate with the care team to implement nursing care plans
Provide clear, timely charting and handoffs to ensure continuity
Support patient comfort, dignity, and education during their stay
Respond quickly to patient needs and escalate concerns appropriately

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania
Graduate of an accredited practical nursing or vocational program
Current BLS/CPR certification or willingness to obtain upon hire
Clean background check and up-to-date immunizations per facility policy
Comfortable working 12-hour shifts and adapting to per diem scheduling
Previous LPN experience in a long-term care or clinical setting preferred

Education

Completion of an accredited LPN or vocational nursing program
High school diploma or equivalent

Knowledge & Skills

Strong medication administration and documentation skills
Solid assessment skills including vital signs and basic patient evaluation
Ability to follow established care plans and facility protocols
Good communication and teamwork with interdisciplinary staff
Basic electronic medical record use and accurate charting
Time management and prioritization during busy shifts
