Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents in a skilled nursing setting
• Administer medications and treatments accurately and safely following facility policies
• Monitor and document resident condition changes and communicate effectively with the interdisciplinary team
• Assist with admission, transfer and discharge processes to ensure continuity of care
• Support resident dignity and independence through daily care, mobility assistance and engagement
• Participate in care planning and follow-through to help achieve resident-centered goals

Qualifications

• Active LPN license in Pennsylvania (or compact/state-appropriate) required
• Current CPR/BLS certification (or able to obtain promptly)
• Comfortable with medication administration, wound care basics and resident assessments
• Ability to pass required background checks, drug screens and facility onboarding
• Strong communication and teamwork skills; a caring attitude is a must
• Previous experience in long-term care or skilled nursing is a plus

Education

• Graduation from an accredited practical nursing program or equivalent diploma/certificate

Knowledge & Skills

• Knowledge of basic nursing procedures and documentation best practices
• Familiarity with medication administration and safety protocols
• Skilled at resident assessment, infection control and skin integrity monitoring
• Ability to prioritize tasks in a fast-paced clinical environment
• Good interpersonal skills for working with residents, families and care teams
