Roles & Responsibilities

• Provide compassionate direct patient care with attention to dignity and comfort
• Administer medications and treatments safely and document accurately
• Perform routine assessments including vital signs and report changes promptly
• Assist with activities of daily living and promote patient independence
• Collaborate with nurses, therapists, and providers to support coordinated care
• Maintain clear, timely charting in the patient record and follow infection control practices
• Support patient and family education with patience and clear communication

Qualifications

• Current Licensed Practical Nurse (LPN) credential — active and in good standing
• Completion of an accredited practical nursing program
• Basic Life Support (BLS) / CPR certification preferred or obtained upon hire
• Comfortable working in a long-term care/center setting and following established protocols
• Reliable, team-oriented attitude with strong communication and time-management skills

Education

• Graduate of an accredited practical nursing program (LPN diploma or certificate)

Knowledge & Skills

• Medication administration and safe documentation
• Patient assessment and monitoring (vitals, skin, mobility)
• Wound care fundamentals and ADL assistance
• Electronic charting / EMR familiarity and accurate record keeping
• Infection control practices and safe patient handling
• Friendly, clear communication with patients, families, and staff
