Roles & Responsibilities

• Administer medications and treatments safely and accurately according to provider orders
• Perform routine assessments and monitor residents' vital signs, reporting changes promptly
• Provide compassionate personal care to residents, promoting dignity and independence
• Document clinical observations and interventions clearly in the medical record
• Collaborate with RNs, therapists, and the interdisciplinary team to implement care plans
• Assist with wound care, dressing changes, and other skilled nursing tasks as needed
• Support resident admissions, discharges, and transitions with attention to detail
• Educate residents and families on care plans, medications, and safety measures

Qualifications

• Active LPN license or eligibility for LPN licensure in New Mexico
• Current BLS/CPR certification or ability to obtain upon hire
• Experience in long-term care, rehab, or skilled nursing preferred
• Ability to work scheduled days: Sunday, Friday, Saturday, including 12+ hour shifts
• Pass background check, drug screen, and required immunizations per facility policy
• Strong communication skills and a resident-centered approach to care

Education

• Completion of an accredited Practical Nursing (LPN/LVN) program
• High school diploma or equivalent

Knowledge & Skills

• Medication administration and accurate charting
• Basic nursing assessments and infection control practices
• Familiarity with electronic medical records and documentation standards
• Time management and prioritization during busy shift workflows
• Team collaboration with nurses, therapists, and support staff
• Compassionate bedside manner and patient education techniques
