Contract Type: Per-Diem

Shift Type: 8-hour shift; specific start/end times and days not specified (schedule is non-guaranteed and set by facility needs)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide direct, compassionate patient care as an LPN, including medication administration and monitoring vital signs
Assist with activities of daily living and ensure patient comfort and safety
Accurately document patient status, interventions, and responses in the medical record
Communicate effectively with RNs, interdisciplinary team members, patients, and families to support coordinated care
Observe and report changes in patient condition promptly to the appropriate clinician
Follow facility policies, infection control practices, and safety procedures to maintain a high standard of care

Qualifications

Valid Licensed Practical Nurse (LPN) license in Pennsylvania or eligibility to obtain one
Current Basic Life Support (BLS/CPR) certification
Completion of an accredited LPN or vocational nursing program
Ability to pass background check, drug screen, and required health screenings
Strong communication skills and a team-oriented attitude
Previous experience in long-term care or skilled nursing is a plus

Education

Completion of an accredited LPN/Vocational Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and documentation
Basic wound care and monitoring
Comfortable taking and interpreting vital signs
Electronic medical record (EMR) familiarity and accurate charting
Patient assessment and reporting changes of condition
Time management and prioritization in a busy clinical setting
