Contract Type: Per-Diem

Shift Type: Per diem, 8-hour shifts (exact start/end times vary by assignment; schedule to be confirmed with facility)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate bedside care and hands-on support to residents
Administer medications and treatments accurately and document thoroughly
Perform focused assessments and promptly report changes in condition
Collaborate with the care team to create and follow individualized care plans
Maintain clear, professional charting and communication with families
Ensure a safe, clean environment and follow infection control policies
Support patient mobility, comfort, and dignity during daily care activities

Qualifications

Active Licensed Practical Nurse (LPN) license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain prior to start)
Previous LPN experience in long-term care or skilled nursing preferred
Ability to work per-diem shifts, including weekends when needed
Strong communication skills and a team-first attitude
Reliable transportation to the facility

Education

Completion of an accredited LPN/Vocational Nursing program
High school diploma or GED

Knowledge & Skills

Medication administration and basic IV/PO knowledge appropriate to LPN scope
Resident assessment and documentation (electronic charting experience a plus)
Infection control standards and safe patient handling
Time management and prioritization in a fast-paced care setting
Compassionate patient-centered care and good interpersonal skills
