Roles & Responsibilities

• Provide skilled nursing care including medication administration and basic wound care per facility protocols
• Monitor resident vital signs and promptly report changes to the care team
• Document assessments, interventions, and resident progress accurately and in a timely manner
• Assist residents with activities of daily living while promoting dignity and independence
• Collaborate with RNs, physicians, therapists, and other staff to implement individualized care plans
• Maintain infection control standards and ensure a safe care environment
• Participate in effective shift handoffs and communicate changes clearly

Qualifications

• Active LPN license in Pennsylvania required
• Current BLS/CPR certification required or obtained upon hire
• Ability to pass background checks and required health screenings (TB, immunizations)
• Per diem availability and flexibility to meet facility staffing needs
• Strong communication skills and a collaborative, patient-centered mindset
• Previous LPN experience in long-term care or skilled nursing is preferred; new graduates from accredited programs are encouraged to apply

Education

• Completion of an accredited Practical Nursing (LPN) program
• High school diploma or GED

Knowledge & Skills

• Medication administration and accurate documentation
• Basic wound care and dressing changes
• Clinical assessment and vital sign monitoring
• Familiarity with infection control and safety protocols
• Comfortable using electronic health records for charting
• Strong observational skills and the ability to prioritize tasks
• Teamwork and effective communication with interdisciplinary staff
