Contract Type: Per-Diem

Shift Type: 8-hour shift, specific start/end times and days to be confirmed (LPN)

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care including routine assessments and bedside care
Administer medications and treatments per physician orders and facility protocols
Accurately document patient status, interventions, and care plans in the chart
Collaborate with the interdisciplinary team to promote patient comfort and recovery
Monitor and report changes in patient condition promptly to nursing leadership
Support residents with activities of daily living while preserving dignity and independence

Qualifications

Current LPN (Licensed Practical Nurse) license — ability to practice in Pennsylvania preferred
Current BLS/CPR certification or willingness to obtain upon hire
Comfortable with medication administration, wound care basics, and routine nursing duties
Strong communication skills and a patient-centered approach
Ability to pass required background checks and health screenings

Education

Completion of an accredited Practical Nursing program (LPN/LVN diploma or certificate)

Knowledge & Skills

Patient assessment and basic clinical monitoring
Medication administration and documentation
Infection control practices and safe patient handling
Electronic charting and attention to detail
Team collaboration and effective communication with residents and families
