Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients
• Administer medications and treatments accurately and safely
• Monitor and record vital signs, reporting changes promptly to the care team
• Document clinical observations and interventions in the patient record
• Assist with activities of daily living and promote patient comfort and dignity
• Collaborate with nurses, therapists, and interdisciplinary staff to deliver coordinated care
• Support care planning and participate in shift handoffs to ensure continuity
• Contribute to a culture of safety, infection control, and quality improvement

Qualifications

• Valid LPN license in Pennsylvania (required)
• Current CPR/BLS certification (or willingness to obtain)
• Prior experience in long-term care, skilled nursing, or similar setting preferred
• Strong communication and interpersonal skills to work with residents and families
• Ability to perform clinical tasks safely and follow facility policies and procedures
• Proof of immunizations and background check as required by facility

Education

• LPN diploma or certificate from an accredited program
• High school diploma or GED

Knowledge & Skills

• Medication administration and documentation best practices
• Basic patient assessment and monitoring
• Wound care fundamentals and infection prevention
• Electronic medical record (EMR) or charting experience
• Time management and prioritization during an 8-hour shift
• Teamwork, problem solving, and clear communication with clinical staff
