Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, high-quality direct patient care to residents
Administer medications and treatments accurately and according to policy
Monitor and document patient status, changes, and communicate those changes promptly
Collaborate with the interdisciplinary care team to support individualized care plans
Assist with admissions, transfers, and discharges in a professional manner
Maintain accurate charting, follow infection control protocols, and uphold facility standards

Qualifications

Active Licensed Practical Nurse (LPN) license (PA license or eligibility to obtain one)
Current CPR/BLS certification (or willingness to obtain promptly)
Ability to pass required background checks and screenings
Strong communication skills and the ability to work well on a team
Previous experience in a long-term care or skilled nursing setting is a plus

Education

Completion of an accredited LPN/Licensed Practical Nurse program
High school diploma or equivalent

Knowledge & Skills

Patient assessment and basic clinical care
Medication administration and documentation
Vital signs monitoring and reporting
Wound care basics and infection control practices
Familiarity with electronic charting / EMR systems
Time management, prioritization, and patient advocacy
