Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in line with care plans
Administer medications and treatments accurately and document thoroughly
Monitor resident condition and report changes to the care team promptly
Collaborate with nurses, therapists, and support staff to ensure coordinated care
Assist with activities of daily living while promoting resident dignity and independence
Participate in care planning and contribute observations that improve outcomes
Maintain accurate, timely charting and follow facility policies and procedures

Qualifications

Current LPN license (eligible or licensed to practice in Pennsylvania preferred)
Basic Life Support (BLS/CPR) certification or willingness to obtain
Comfort with medication administration, documentation, and resident care
Ability to pass background check and meet facility onboarding requirements
Strong communication skills and a teamwork-oriented attitude
Reliable attendance and professional bedside manner

Education

LPN diploma, certificate, or equivalent practical nursing program completion

Knowledge & Skills

Knowledge of nursing fundamentals and long-term care clinical practices
Familiarity with medication administration and basic charting systems
Skills in vital signs assessment, wound care basics, and resident observation
Ability to prioritize tasks in a busy clinical setting
Strong interpersonal skills for resident and family interactions
