Roles & Responsibilities

Provide compassionate, patient-centered nursing care within scope of practice
Administer medications and treatments accurately and document thoroughly
Monitor and record vital signs, reporting changes in condition promptly
Assist with ADLs and support patients' comfort and dignity
Collaborate with RNs, physicians, and interdisciplinary team to coordinate care
Maintain accurate and timely electronic health record documentation
Follow infection control protocols and facility policies to promote safety
Participate in shift handoffs and communicate clear updates to team members

Qualifications

Valid Licensed Practical Nurse (LPN) license in Pennsylvania - required
Current BLS/CPR certification - required
Previous experience in long-term care, skilled nursing, or acute care preferred
Strong clinical judgment, attention to detail, and compassionate bedside manner
Ability to pass facility onboarding and background checks
Immunizations and health screenings per facility policy

Education

Graduate of an accredited Practical Nursing program (LPN diploma or certificate)

Knowledge & Skills

Medication administration and basic clinical procedures
Comfortable with electronic health record documentation
Skilled in patient assessment and recognizing changes in condition
Knowledge of infection control and safe patient handling
Clear communication and teamwork with interdisciplinary staff
Time management and prioritization in a fast-paced setting
