Contract Type: Per-Diem

Shift Type: 7-hour shift, specific start/end times and days to be confirmed

Hours per Week: 40

Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate, skilled nursing care to long-term care residents
Administer medications and treatments accurately and document thoroughly
Monitor resident condition and communicate changes to the care team
Support activities of daily living and promote resident dignity and independence
Participate in care planning and follow through on individualized care goals
Collaborate with CNAs, RNs, and interdisciplinary team members to deliver cohesive care
Maintain accurate, timely documentation and follow facility policies and procedures

Qualifications

Active LPN license in Pennsylvania (or compact/state-appropriate license)
Current CPR/BLS certification or willingness to obtain upon hire
Comfortable working in a long-term care environment and advocating for residents
Strong communication skills and a team-first attitude
Previous experience in long-term care or skilled nursing is preferred

Education

Practical nursing diploma or certificate (LPN) from an accredited program

Knowledge & Skills

Solid clinical skills in assessments, medication administration, and wound care
Familiarity with electronic health records and documentation best practices
Ability to prioritize tasks in a fast-paced, resident-focused setting
Compassionate bedside manner and strong interpersonal skills
Attention to detail and commitment to patient safety and quality
