Roles & Responsibilities

Provide compassionate direct patient care and daily living assistance
Administer medications and treatments per care plan and scope
Monitor and document patient vital signs, changes, and responses
Collaborate with RNs and interdisciplinary team to deliver quality care
Maintain accurate, timely charting in the facility’s documentation system
Support safe patient transfers, mobility assistance, and fall prevention
Communicate clearly with residents and families to promote comfort and understanding

Qualifications

Active LPN license in Pennsylvania (required)
Current BLS/CPR certification (or willingness to obtain)
Hands-on experience in long-term care or skilled nursing preferred
Ability to pass background check and required immunizations
Strong communication skills and a patient-centered mindset

Education

Completion of an accredited Practical Nursing (LPN/LVN) program or equivalent vocational certificate

Knowledge & Skills

Medication administration and basic clinical assessments
Familiarity with long-term care workflows and resident care needs
Comfortable with documentation and electronic charting systems
Good clinical judgment, organization, and time management
Teamwork and clear communication with clinical staff and families
