Candidate Pay Rate: $34 per hour

Roles & Responsibilities

Provide compassionate direct patient care consistent with LPN scope of practice
Administer medications and treatments accurately and document appropriately
Monitor patient condition and communicate changes to the care team
Assist with daily living activities and promote patient dignity and comfort
Collaborate with RNs and interdisciplinary team members to support care plans
Maintain accurate and timely documentation in the patient record
Support a safe, clean environment and follow infection control protocols

Qualifications

Active LPN (Licensed Practical Nurse) license in Pennsylvania or compact state as applicable
Current BLS/CPR certification (or willingness to obtain upon hire)
Prior experience in long-term care, skilled nursing, or similar clinical setting preferred
Strong communication and teamwork skills with patient-centered focus
Ability to pass background checks and required facility screenings

Education

Completion of an accredited LPN/Licensed Practical Nurse program
High school diploma or equivalent

Knowledge & Skills

Knowledge of basic nursing principles and medication administration
Familiarity with patient assessment and documentation practices
Ability to follow care plans and escalate clinical concerns appropriately
Comfortable working in a fast-paced, collaborative healthcare setting
Strong interpersonal skills and a respectful bedside manner
