Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a long-term care setting
Administer medications and treatments accurately and document all care
Assess resident condition and report changes promptly to the care team
Collaborate with nurses, therapists and support staff to create meaningful care plans
Assist with activities of daily living while promoting dignity and independence
Communicate clearly and kindly with residents and their families to keep them informed
Maintain clinical documentation and follow facility policies and safety procedures

Qualifications

Current LPN license in good standing (Connecticut license recommended)
Completion of an accredited practical nursing program
Basic Life Support (BLS/CPR) certification or willingness to obtain
Comfortable working in a long-term care or skilled nursing environment
Strong communication skills and a collaborative mindset
Prior LPN experience preferred but new grads from an LPN program are welcome to apply

Education

Graduate of an accredited LPN / Practical Nursing program
High school diploma or GED required

Knowledge & Skills

Medication administration and documentation best practices
Vital signs assessment and basic clinical observations
Infection control and HIPAA-compliant record keeping
Patient-centered care and effective communication with families
Time management and the ability to prioritize resident needs
