Roles & Responsibilities

• Provide compassionate, hands-on nursing care to residents and patients consistent with LPN scope
• Administer medications and document accurately to ensure safe, effective treatment
• Monitor patient status and communicate changes to RN and clinical leadership promptly
• Assist with ADLs and support patient comfort, dignity, and mobility
• Collaborate with interdisciplinary team members to deliver coordinated care
• Maintain accurate clinical records and follow facility policies and procedures
• Support infection control practices and a safe care environment

Qualifications

• Active LPN license in Pennsylvania (required)
• Graduate of an accredited Practical Nursing / LPN program
• Current BLS/CPR certification (or willingness to obtain upon hire)
• Strong clinical assessment and medication administration skills
• Effective communication skills and ability to work as part of a team
• Previous experience in long-term care, skilled nursing, or related setting is a plus

Education

• Completion of an accredited LPN / Practical Nursing program (diploma or certificate)

Knowledge & Skills

• Medication administration and charting best practices
• Basic wound care and monitoring of vital signs
• Patient safety, infection control, and fall prevention
• Electronic health record documentation
• Time management and prioritization in a busy clinical setting
• Empathy, strong interpersonal skills, and resident-centered care
