Roles & Responsibilities

• Conduct thorough psychosocial assessments to understand patients across the lifespan
• Develop individualized, evidence-based treatment plans tailored to each person
• Deliver therapeutic interventions for depression, anxiety, trauma, substance use and adjustment disorders
• Collaborate closely with primary care providers, nurses, and behavioral health colleagues for coordinated whole-person care
• Maintain accurate, timely clinical documentation to support continuity and quality of care
• Coordinate referrals and community resources to support underserved populations
• Advocate for patient needs and contribute to a culture of compassionate, equitable care

Qualifications

• Active LISW license in Iowa (required)
• Master's degree in Social Work from an accredited program (MSW)
• Minimum of 2 years of clinical social work experience preferred
• Experience working in rural or medically underserved settings is a strong plus
• Strong interpersonal and organizational skills with excellent clinical documentation abilities
• Comfort collaborating within integrated care teams and coordinating patient care

Education

• Master's degree in Social Work (MSW) from an accredited program

Knowledge & Skills

• Therapeutic interventions across the lifespan
• Clinical documentation best practices and electronic health record use
