Roles & Responsibilities

• Assess patients and caregivers using the nursing process to develop and update individualized, evidence-based care plans
• Serve as case manager for assigned patients, managing caseloads, visit timing, and coordination across disciplines
• Complete accurate, timely OASIS and agency documentation in accordance with state and federal requirements
• Implement medical plans of care and nursing interventions to ensure safe, high-quality hospice services
• Provide high-tech clinical skills at the bedside and in the home, including IV therapy, wound VAC and complex wound care
• Maintain infection control standards across the plan of care and act quickly on incident reporting
• Educate patients and caregivers on medications, symptom management, discharge instructions and care options
• Coordinate care and communicate patient needs to physicians, therapists, social workers and other team members
• Participate in interdisciplinary team (IDT) meetings, peer reviews and case conferences to improve outcomes

Qualifications

• Active Registered Nurse (RN) license — Vermont State Board of Nursing (required)
• Experience in hospice and/or home health nursing with demonstrated case management skills
• Proven ability to complete OASIS documentation accurately and on time
• Hands-on experience with IV therapy, wound VAC, and complex wound care
• Comfortable performing occasional home visits and coordinating with community providers
• Familiarity with McKesson workflows and electronic visit documentation

Education

• Active Registered Nurse (RN) license (Vermont State Board of Nursing)

• Hospice care principles and palliative symptom management
• Home health nursing practices and OASIS documentation standards
• IV therapy maintenance, wound care best practices, and wound VAC management
• Infection control procedures and regulatory compliance (state and federal)
• Patient and caregiver education techniques for informed decision-making
• Interdisciplinary team coordination and discharge planning
• Clinical documentation excellence and use of McKesson systems
