Shift Type: Monday-Friday, 7:30 AM - 4:00 PM (Day); 5x8-hour schedule. Weekend requirement: every other weekend or 4 weekend days in a 4-week schedule. May float within case management services to cover gaps.

Roles & Responsibilities

• Coordinate patient care across the continuum to ensure smooth transitions and timely discharges
• Perform utilization review using Milliman Guidelines to support appropriate level of care
• Develop and maintain individualized plans of care and manage care coordination activities
• Conduct needs assessments and arrange durable medical equipment and community resources
• Document thoroughly and accurately in Epic to support regulatory compliance and continuity of care
• Collaborate with interdisciplinary teams including social services, pharmacy, PT/OT, and specialty services
• Provide coverage across case management services as needed to meet facility demands

Qualifications

• AK Registered Nurse license in hand required for auto-offer
• Minimum 1 year acute care nursing experience
• Epic EMR experience required
• ACM certification preferred
• BLS certification preferred (facility notes indicate preference)
• Willingness to work required weekend rotation (every other weekend or 4 weekend days/4-week schedule)
• Candidates must have permanent address at least 50 miles from facility (locals not accepted)

Knowledge & Skills

• Strong case management and utilization review skills
• Discharge planning and care coordination expertise
• Familiarity with Milliman Guidelines
• Proficient Epic charting skills, including Epic hardstop workflows
• Comfortable collaborating with multidisciplinary teams
• Proficient with Microsoft Office and Teams for documentation and communication
