Roles & Responsibilities
- Provide compassionate, high-quality direct patient care to residents
- Administer medications and treatments according to orders and facility policy
- Perform accurate patient assessments and report changes promptly
- Document care clearly and timely in the patient record
- Collaborate with nursing staff and interdisciplinary team to support resident goals
- Support admissions, discharges and smooth transitions of care
- Maintain a safe, clean environment and follow infection control practices
Qualifications
- Current, active LPN license for Massachusetts (or RN license if applying as RN)
- BLS/CPR certification required (state or employer requirements may apply)
- Comfortable working per diem/PRN shifts and adapting to facility needs
- Experience in long-term care, rehab, or skilled nursing settings is a plus
- Strong communication skills and ability to work collaboratively
Education
- LPN: Vocational nursing diploma or equivalent
- RN applicants: Associate Degree or diploma in Nursing (if applying as RN)
Knowledge & Skills
- Patient assessment and vital signs monitoring
- Medication administration and basic pharmacology knowledge
- Wound care basics and dressing management
- Proficient documentation and charting (electronic or paper)
- Infection control and safety-focused practices
- Effective teamwork, time management, and resident-centered care
