Roles & Responsibilities

• Provide compassionate hands-on nursing care to residents, focusing on safety and dignity
• Administer medications and treatments accurately, following facility policy and provider orders
• Perform timely and thorough patient assessments and document findings clearly
• Collaborate with interdisciplinary team members to develop and update individualized care plans
• Monitor changes in condition and escalate concerns to nursing leadership or providers
• Support residents with activities of daily living while promoting independence and comfort
• Participate in shift handoffs to ensure continuity of care and accurate information transfer

Qualifications

• Active LPN or RN license in Massachusetts required
• Current BLS (Basic Life Support) certification required or willing to obtain
• Previous experience in long-term care, rehab, or skilled nursing preferred but enthusiastic new grads are welcome
• Strong communication skills and a patient-centered approach
• Able to pass facility background and health screenings

Education

• LPN: Completion of an accredited LPN/LVN program
• RN: Associate Degree in Nursing (ADN) or Bachelor of Science in Nursing (BSN)

Knowledge & Skills

• Clinical assessment and observation skills
• Medication administration and documentation best practices
• Familiarity with care planning and charting in a skilled nursing setting
• Ability to prioritize tasks in a fast-paced environment
• Teamwork, empathy, and clear communication with residents and families
