Roles & Responsibilities

Provide compassionate, hands-on nursing care to residents in a skilled nursing and rehab setting
Administer medications and treatments safely and accurately according to orders
Perform routine assessments and monitor changes in patient condition
Collaborate with RNs, therapists, and interdisciplinary team members to develop and follow care plans
Document care thoroughly and timely in the facility charting system
Assist with activities of daily living and promote resident dignity and independence
Support patient transitions and communicate effectively with families and care partners

Qualifications

Current Licensed Practical Nurse (LPN) license in Massachusetts required
Basic Life Support (BLS) CPR certification required
Experience in long-term care or rehab settings preferred but eager learners encouraged to apply
Current immunizations and ability to pass required background and health screenings
Strong communication skills and a team-oriented attitude

Education

Graduate of an accredited LPN/LVNs program or equivalent
Current LPN license in Massachusetts

Knowledge & Skills

Medication administration and documentation best practices
Basic wound care and restorative nursing techniques
Skilled nursing assessment and reporting of changes in condition
Familiarity with electronic charting or willingness to learn quickly
Ability to prioritize, multitask, and maintain a calm, patient-centered approach
