Roles & Responsibilities

• Provide compassionate, patient-centered physical therapy care in patients' homes
• Perform thorough assessments and develop individualized treatment plans
• Deliver skilled interventions focused on mobility, balance, strength, and safety
• Educate patients and caregivers on home exercise programs and fall prevention
• Document clinical findings and progress accurately in the electronic medical record
• Collaborate with nursing, social work, and interdisciplinary teams to support discharge goals
• Complete required competency testing and maintain compliance with agency policies

Qualifications

• Active Physical Therapist (PT) license for Iowa and verification of credentials
• Basic Life Support (BLS) certification required
• Home health experience preferred and comfort working autonomously in the field
• Epic experience preferred but not required
• Successful completion of Trinity Specialty Specific Competency Testing and Skills Checklist
• Compliance with required health screenings: fit test, drug screen, MMR, Hep B, TB testing, Varicella, Tdap, COVID-19 and influenza vaccination documentation

Knowledge & Skills

• Strong assessment skills for mobility, balance, transfer training, and fall risk
• Proficient clinical documentation and charting in electronic medical records
• Skilled in patient and caregiver education with clear, encouraging communication
• Ability to plan safe, practical home exercise programs and home safety modifications
• Good time management and caseload prioritization for efficient in-home visits
• Familiarity with infection control practices and community-based care protocols
