Roles & Responsibilities

• Coordinate patient care and transitions to home, home health, SNF, LTAC, hospice and other settings
• Conduct thorough psychosocial assessments to inform safety planning and care needs
• Develop and implement individualized discharge plans in collaboration with clinical teams
• Assist with complex placements and address bio-psychosocial barriers to safe discharge
• Collaborate closely with physicians, nurses, case managers and community partners to connect patients with resources
• Document care plans and patient interactions accurately in MEDITECH
• Participate in utilization review and care management activities as required
• Support timely, patient-centered decision making that improves outcomes and reduces readmissions

Qualifications

• Clinical Social Work License (LCSW) in the state of Oregon — required
• Minimum 2 years of acute hospital experience preferred
• BLS (Basic Life Support) certification required
• Experience with discharge planning, care coordination and case management
• Familiarity with MEDITECH or similar EMR systems
• Strong assessment, communication and collaboration skills
• Certifications such as CCM, ACM, C-SWCM, or CBHCM are a plus but not required

Education

• Clinical Social Work License (LCSW) - Oregon (required)

Knowledge & Skills

• Care coordination and transitions of care processes
• Discharge planning best practices for inpatient and ER settings
• Psychosocial assessment and safety planning
• Case management and utilization review principles
• Experience using MEDITECH EMR for documentation and care coordination
• Community resource navigation and partner collaboration
• Ability to manage a caseload of ~15-20 consults per day with prioritization skills
