Roles & Responsibilities

Evaluate patients and develop individualized occupational therapy treatment plans that promote independence
Implement therapeutic interventions to improve ADLs and functional mobility
Collaborate with an interdisciplinary rehab team to support safe, effective patient care
Educate patients and caregivers on safety, exercises, and adaptive techniques to empower recovery
Document clinical findings and progress accurately and timely to support continuity of care

Qualifications

Active California Occupational Therapist (OT) license required
SNF or rehab experience preferred
Strong clinical reasoning and communication skills to drive patient-centered care
Ability to work independently and as part of a collaborative team
Comfortable with timely and accurate documentation practices

Knowledge & Skills

Patient evaluation and treatment planning
Clinical reasoning and evidence-based interventions
ADLs and functional mobility training
Interdisciplinary collaboration and teamwork
Patient and caregiver education
Accurate clinical documentation
