Roles & Responsibilities

Assist the Occupational Therapist in implementing individualized treatment plans to help residents regain independence
Provide therapeutic interventions to improve activities of daily living (ADLs) and functional mobility
Educate patients and families on exercises, safety strategies, and adaptive techniques to support recovery
Collaborate closely with the interdisciplinary rehab team to coordinate care and maximize outcomes
Complete accurate and timely documentation to support patient progress and regulatory compliance

Qualifications

Active California COTA license (required)
Experience in Skilled Nursing Facility (SNF) or rehab settings preferred
Strong clinical skills with demonstrated ability to support OT interventions
Excellent verbal and written communication skills
Able to work independently and as part of a team

Knowledge & Skills

Understanding of ADLs, functional mobility, and adaptive equipment
Patient education and safety instruction
Accurate clinical documentation and progress note writing
Interdisciplinary team collaboration
Strong communication and teamwork abilities
