Roles & Responsibilities

• Evaluate and screen patients with communication, cognitive, or swallowing disorders and develop individualized plans of care
• Use standardized assessments and evidence-based interventions to guide treatment
• Monitor patient progress and adjust treatment plans, documenting changes in the electronic health record
• Assess treatment outcomes and make referrals based on patient progress
• Provide counseling and education to patients, families, caregivers, and interdisciplinary team members
• Prepare comprehensive discharge summaries that meet regulatory and clinical standards
• Accurately document treatment minutes and all required clinical information in the EHR
• Participate in patient care conferences, utilization review, and rehabilitation meetings
• Supervise and support CFY and student SLPs, offering mentorship and clinical oversight
• Engage in facility and company trainings, in‑services, and continuing education activities

Qualifications

• Active CCC (ASHA) required
• Texas State License required (Compact allowed)
• Minimum 1 year of SLP experience (LTC or Rehab experience required)
• New grads accepted if they have Level II clinical experience (Level I observation alone is not sufficient)
• Ability to supervise CFY and student SLPs
• Familiarity with EHR documentation and regulatory/compliance requirements
• Will accept one reference at time of submission
• CCS credential preferred (3 years experience holding active CCS preferred)

Knowledge & Skills

• Evaluation and assessment of speech, language, cognition and swallowing (dysphagia)
• Evidence-based clinical interventions and standardized assessments
• Clinical documentation, regulatory compliance, and treatment outcome evaluation
• Treatment planning and discharge planning
• Accurate recording of treatment minutes and EHR proficiency
• Patient, family and caregiver counseling and education
• Supervision, mentorship and collaboration with interdisciplinary teams
• Ability to obtain necessary DME and supplies when required
