Hours per Week: 40
Candidate Pay Rate: $74 per hour

Roles & Responsibilities
- Provide compassionate care coordination for patients in need of mental health support
- Complete thorough assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient care
- Support patients and families through the 5150 evaluation process when needed
- Document all clinical interactions accurately and timely
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities required
Knowledge & Skills
- Strong assessment and crisis intervention skills
- Knowledge of involuntary hold procedures and mental health regulations
- Excellent communication and documentation abilities
- Ability to work collaboratively within a care coordination team
