Contract Type: Contract

Shift Type: Monday-Friday, 7:30 AM - 4:00 PM or 8:00 AM - 4:30 PM, up to every other weekend

Hours per Week: 40

Candidate Pay Rate: $71 per hour

Roles & Responsibilities

Manage a meaningful caseload of 5-6 new patients and 3-4 follow-ups daily
Lead compassionate goals-of-care discussions with patients and families
Educate patients on disease processes, advance directives, and POLSTs
Facilitate family meetings and provide end-of-life support
Collaborate during daily rounds with physicians and the interdisciplinary team
Review patient charts and assess needs across multiple hospital units

Qualifications

Active California RN license
BLS certification required
ELNEC certification preferred
2-5 years recent palliative or hospice experience
Strong acute care, clinical thinking, and communication skills
Flexibility and excellent organizational abilities

Knowledge & Skills

Palliative and hospice care expertise
Cerner EMR proficiency
Goals-of-care and end-of-life communication
Strong organizational and time-management skills
Compassionate approach to serving patients and families
