Shift Type: Monday-Friday, 8:00 AM - 5:00 PM, every other weekend

Hours per Week: 40

Candidate Pay Rate: $82 per hour

Roles & Responsibilities

Coordinate care across all hospital areas to ensure seamless patient experiences
Complete initial assessments and utilization reviews using Interqual criteria
Develop comprehensive discharge plans that support patients and families
Collaborate with interdisciplinary teams to optimize patient outcomes
Navigate payer requirements while advocating for patients in need
Support diverse populations including uninsured and homeless individuals

Qualifications

Active California RN license
BLS certification required
Prior recent experience with Dignity Health facilities
Strong collaboration and communication skills
Ability to adapt and reprioritize throughout the day

Knowledge & Skills

Case Management and Care Coordination expertise
Utilization Review and Interqual criteria knowledge
Cerner electronic medical record proficiency
Discharge planning across all payer types
Teamwork and effective communication abilities
