Roles & Responsibilities

• Assess speech, language, cognition and swallowing needs for skilled nursing residents and develop individualized treatment plans
• Deliver evidence-based SLP interventions within the SNF setting to improve patient function and quality of life
• Document evaluations, plans of care, daily treatment notes and discharge plans in a timely, compliant manner
• Collaborate closely with nurses, therapists and interdisciplinary team members to coordinate comprehensive care
• Meet facility productivity expectations (therapist productivity requirement: 90%) while maintaining high-quality outcomes
• Adhere to Continuum Rehabilitation Group (CRG) policies & procedures and facility protocols

Qualifications

• 1–3 years of SLP experience (as stated in the job details)
• Current credentials and ability to practice as an SLP in New Mexico (state licensure requirements apply)
• Comfort working in a Skilled Nursing Facility (SNF) practice setting
• Willingness to follow CRG policies & procedures and meet productivity standards
• Ability to be present on-site (therapist must be in the building to receive guaranteed hours) and to follow facility dress code (black scrubs)
• Professional communication skills and ability to collaborate with an interdisciplinary team

Knowledge & Skills

• Expertise in Speech-Language Pathology within a SNF setting — assessment and intervention for adults
• Strong documentation practices and familiarity with productivity expectations in contract therapy
• Ability to create patient-centered treatment plans and track measurable outcomes
• Team-oriented approach to coordinate care with nurses and other therapists
• Comfort with facility policies and the ability to follow established protocols
