Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:00 AM - 4:30 PM with rotating weekends

Hours per Week: 40

Candidate Pay Rate: $56 per hour

Roles & Responsibilities

Coordinate seamless discharge planning for patients transitioning from acute care
Collaborate with physicians, social workers, and care teams to ensure optimal patient outcomes
Advocate for patients and families throughout their healthcare journey
Utilize Meditech, Midas, and Wellsky systems to document and track care plans
Support patients across a 590-bed facility with varying patient ratios

Qualifications

Active RN license in Missouri or compact state
Minimum 1 year of recent acute care discharge planning experience
Strong communication and organizational skills
Ability to work collaboratively within a multidisciplinary team

Knowledge & Skills

Discharge Planning expertise in acute care settings
Familiarity with Meditech, Midas, and Wellsky documentation systems
Understanding of patient experience best practices
Flexibility to float to various units as needed
