Roles & Responsibilities
- Provide compassionate care coordination for patients in need of mental health support
- Conduct thorough assessments and develop individualized care plans
- Collaborate with interdisciplinary teams to ensure seamless patient transitions
- Support patients and families through the 72-hour involuntary evaluation process when appropriate
- Maintain accurate documentation and advocate for patient rights and safety
Qualifications
- Active California LCSW license required
- Current BLS certification
- 5150 certification in California required
- Prior experience with Dignity Health facilities strongly preferred
- Strong assessment and crisis intervention skills
Knowledge & Skills
- Expertise in involuntary psychiatric holds and mental health crisis response
- Excellent communication and documentation abilities
- Ability to work collaboratively within a multidisciplinary team
- Knowledge of California mental health regulations and patient advocacy
