Roles & Responsibilities
- Deliver compassionate physical therapy care to patients in their homes across greater Sacramento county
- Treat a diverse caseload including cardio, pulmonary, orthopedic, neurological, and geriatric patients
- Help patients regain strength, mobility, and independence with personalized treatment plans
- Collaborate with interdisciplinary teams to ensure the best outcomes for every patient
- Document visits accurately and maintain compliance with all regulatory standards
Qualifications
- Active Physical Therapy Assistant license in California
- Prior home health experience preferred
- New grads considered only in exceptional circumstances
- Strong communication and time-management skills
- Reliable transportation for home visits
