Roles & Responsibilities
- Provide compassionate physical therapy services to patients in their homes across Yolo County
- Assess and evaluate each patient's therapeutic needs and develop personalized care plans
- Guide patients and families through rehabilitation exercises and the proper use of therapeutic equipment
- Monitor progress and communicate updates with physicians to ensure optimal outcomes
- Collaborate with home health aides and caregivers to support each patient's rehabilitation goals
- Document clinical findings and progress summaries to support quality care and compliance
Qualifications
- Minimum of 2 years experience as a Physical Therapist in acute, home care, or rehab settings
- Current BLS certification required
- Strong communication and organizational skills
- Ability to work independently while providing excellent patient-centered care
Knowledge & Skills
- Physical Therapy assessment and treatment planning
- Home environment evaluation and safety awareness
- Patient and family education techniques
- Documentation and clinical reporting
- Understanding of Medicare and Medi-Cal regulations
