Contract Type: Travel Contract

Shift Type: Monday-Friday, 8:30 AM - 5:00 PM

Hours per Week: 40

Candidate Pay Rate: $53 per hour

Roles & Responsibilities

Coordinate seamless care transitions for patients across the healthcare continuum
Collaborate with physicians, nurses, and social workers to develop comprehensive discharge plans
Advocate for patients while ensuring appropriate resource utilization and length of stay
Utilize Midas and Meditech systems to document and track case management activities
Facilitate complex discharge planning to support safe patient transitions home or to next level of care

Qualifications

Active RN license in Florida or compact license
Minimum 1 year of case management experience
Midas experience required
Meditech experience required
Strong communication and organizational skills

Knowledge & Skills

Proficient with Midas and Meditech electronic systems
Expertise in complex discharge planning processes
Knowledge of healthcare regulations and resource management
Ability to work collaboratively within interdisciplinary teams
