Roles & Responsibilities
- Conduct timely post-hospital and ED follow-up phone calls (including 48-hour TCM calls) to ensure safe transitions
- Apply case management principles to acute care patients to support appropriate utilization and level of care
- Facilitate discharge planning and coordinate referrals to skilled nursing, home health, outpatient providers, and infusion therapy
- Identify barriers to patient flow and collaborate with interdisciplinary teams to resolve them
- Recognize patients who need Case Management or Social Service referrals and initiate appropriate interventions
- Complete accurate, timely documentation in Epic and Cerner to support continuity of care
- Serve as a resource to the ED Hospital Liaison and support effective communication across teams
- Participate in process improvement efforts to reduce unnecessary admissions, lower readmission rates, and improve flow metrics
- Maintain patient confidentiality and HIPAA compliance at all times
Qualifications
- Active, unencumbered Registered Nurse (RN) license in Rhode Island (required)
- Minimum 5 years of related clinical experience in Acute Care or Case Management
- BLS certification (required)
- Med/Surg RN background strongly preferred
- Knowledge of utilization review, discharge planning, and third-party payor regulations
- Strong interpersonal, oral, and written communication skills
- Demonstrated analytical ability, leadership, and effectiveness working in teams
Education
- Bachelor’s Degree in Nursing (BSN) preferred; equivalent experience accepted
Knowledge & Skills
- Telephonic case management and Transitions of Care expertise
- Documentation proficiency in Epic and Cerner
- Utilization review and discharge planning processes
- Familiarity with third-party payor requirements and NCQA timeliness benchmarks
- HIPAA compliance and patient confidentiality best practices
- Excellent communication, care coordination, and documentation accuracy
- Analytical thinking and ability to support process improvement initiatives
